{"created":"2023-05-15T11:33:37.868273+00:00","id":4126,"links":{},"metadata":{"_buckets":{"deposit":"7b81a228-5902-4be5-a2ec-d4e5b07ba5ca"},"_deposit":{"created_by":1,"id":"4126","owners":[1],"pid":{"revision_id":0,"type":"depid","value":"4126"},"status":"published"},"_oai":{"id":"oai:nsg.repo.nii.ac.jp:00004126","sets":["6:10:13"]},"author_link":["15725","15726","15728","15727"],"item_19_biblio_info_8":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2009-01","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"84","bibliographicPageStart":"73","bibliographicVolumeNumber":"20","bibliographic_titles":[{"bibliographic_title":"医療経済研究"}]}]},"item_19_description_16":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_19_description_45":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"subitem_description":"論文(Article)","subitem_description_type":"Other"}]},"item_19_description_6":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"近年、医療経済学の分析手法の中でも、費用効用分析を用いた研究報告が増えている。費用効用分析ではQuality-Adjusted Life Years (QALYs) を用いるが、この算出に必要なのが健康効用値である。健康効用値の評価には直接法と間接法があり、最近は標準化された質問票を用いた間接法による多属性健康効用値評価が増えている。しかしながら、本邦においてはそのデータ蓄積が十分でなく、評価用具の妥当性の検討も課題として残されている。本研究では回復期リハビリテーション実施患者を対象に健康効用値の変化を調べ、その有用性や妥当性を検討した。健康効用値の評価には日本語版HealthUtilities Index Mark3 (HUI3) を用い、その測定用具としての妥当性もあわせて検討した。対象患者は全国の5つ病院の回復期リハビリテーション病棟に入院する脳血管障害や大腿骨頭部骨折などの患者521例である。全体では健康効用値は入院時に0.10であったものが、退院時には0.33に改善し、その増分は0.22となった。診断の遣いによる増分の差は認められなかった。また、HUI3のsingle scoreの比較では、移動領域、認知領域が低くなり、それぞれ入院時に0.31、0.61、退院時には0.57，0.69であった。診断を間わず入院期間に改善を示した領域は移動領域と感情領域のみであった。singlescore聞の相関では、感情と痛み(r=0.510)、移動認知(r=0.508)、会話認知(r=0.470) などで相関を認めた一方で、視覚と器用さ(r=0.020)、聴覚と器用さ(r=0.041)、視覚と痛み(r=0.065) で相闘を認めなかった。また、ADLの指標であるBarthel Index との相聞は、r=0.724~0.768 (p<0.001) となった。HUI3によって評価された健康効用値は転帰の群ごとによる比較でもBIと同様の特徴が認められ、健康効用値がリハビリテーションのアウトカム指標として一定の有用性があることが確認できた。また、日本語版HUI3については、構成概念妥当性などが確認され、今後の医療経済学的な分析に用いることが可能であると示唆された。","subitem_description_type":"Abstract"},{"subitem_description":"In recent years，t he study which used cost utility analysis is increasing. In cost utility analysis，quality-adjusted life years (QALYs) which is calculated by health utility score is used. For measuring the health utility score，there are direct method and an indirect method. Recently，it is often used the multi-attribute healthy status classification systems in an indirect method. However，in Japan，the data accumulation and examination of the validity of the instrument is not enough. This study investigated change of the health utility score for the sub-acute rehabilitation patient，and examined the validity of the Japanese version Health Utilities Index Mark3 (HUI3). A total of 521 patients hospitalized in the sub-acute rehabilitation ward of 5hospitals，such as cerebrovascular disorder and hip fracture completed the HUI3. Mean utility score was 0.10 at hospitalization，0.33 at leaving hospital respectively，and the improvement difference was 0.22. Moreover，in comparison of single score of HUI3，the ambulation attribute and the cognition attributebecame low and were 0.31，0.61at the hospitalization，and 0.57，0.69 at the leaving hospital respectively. The attribute which showed the improvement regardless of diagnosis at duration of hospitalization were only ambulation and emotion. Moreover，the correlation between health utility score measured by HUI3 and Barthel Index was r= 0.724-0.768 (p< 0.001). These data indicate the health utility score measured by HUI3 had usefulness as an outcome index for rehabilitation.
