 note = {application/pdf, 論文(Article), 本研究は、都道府県が医療法に基づき策定する地域医療構想における地域包括ケアシステムの位置づけを検討することを目的とした。地域医療構想の策定については、厚生労働省医政局長から各都道府県知事宛てに「地域医療構想策定ガイドライン」が通知されており、各都道府県の地域医療構想においては地域包括ケアシステムの位置づけとその取扱が明確化されていると考えた。47都道府県の地域医療構想を対象にして、策定趣旨、市町村介護保険事業計画との整合性、地域包括ケアシステムの構築に向けた基本理念、地域包括ケアシステムの構築に向けての取組等を分析した結果、各都道府県における考え方や取扱に大きな違いがあることが明らかにされた。2003年6 月の高齢者介護研究会報告書で地域包括ケアシステムの構想が提唱されてから15年が経過し、社会保障改革プログラム法（2013年12月）で地域包括ケアシステムが規定されてから5 年になろうとしている。このような状況下で、地域医療構想における地域包括ケアシステムの位置づけとその取扱がリアリティを欠いていることは研究課題の一つとなる。, The present study considers the positioning and handling of integrated community-based care systems within the regional medical visions being formulated by Japanese prefectures based on the Medical Care Act. Regional Medical Vision Guidelines have been issued by the Director General, Health Policy Bureau of the Ministry of Health, Labour and Welfare to the governors of each prefecture, and it was thought that the positioning and handling of integrated community-based care systems would be clarified within the regional medical visions of each prefecture. Analysis of regional medical visions for 47 prefectures in terms of factors including formulation intent, consistency with municipal long-term care insurance business plans, guiding principles behind the construction of integrated community-based care systems, and engagement toward the construction of integrated community-based care systems revealed major differences in the thought processes and approaches of individual prefectures. Fifteen years have now passed since the concept of integrated community-based care systems was proposed in the June 2003 report issued by the Elderly Persons Care Research Group, and it has been almost five years since integrated community-based care systems were defined in the Reform Promotion Act for the Establishment of a Sustainable Social Security System（ December 2013）. Given these circumstances, the lack of realism in the positioning and handling of integrated community-based care systems within regional medical visions represents an important research challenge.},
