{"created":"2023-05-15T11:34:26.618480+00:00","id":5017,"links":{},"metadata":{"_buckets":{"deposit":"ea23d033-28fc-4add-8930-f7bb4317e887"},"_deposit":{"created_by":11,"id":"5017","owners":[11],"pid":{"revision_id":0,"type":"depid","value":"5017"},"status":"published"},"_oai":{"id":"oai:nsg.repo.nii.ac.jp:00005017","sets":["6:10:11:66"]},"author_link":["18670","18668","18665","18666","18667","18669"],"item_19_biblio_info_8":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2019-11-29","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"3","bibliographicPageEnd":"113","bibliographicPageStart":"107","bibliographicVolumeNumber":"19","bibliographic_titles":[{"bibliographic_title":"新潟医療福祉学会誌"}]}]},"item_19_description_16":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_19_description_45":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"subitem_description":"論文(Article)","subitem_description_type":"Other"}]},"item_19_description_6":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【目的】回復期リハビリテーション(以下、回復期リハ)患者の入棟時において、栄養状態の低下を惹起する口腔内の要因を明らかにする。【方法】2017年8 月~2018年7 月、総合リハビリテーションセンターみどり病院の回復期リハ病棟に入棟した65歳以上の65名を対象とし、入棟時の栄養状態と口腔機能の関連について検討した。入棟時、Geriatric Nutritional Risk Index(GNRI)を用いて栄養評価を行い、GNRI <92は、栄養状態不良(以下、不良群)、GNRI≧92は、栄養状態良好(以下、良好群)として分類した。栄養状態をOral Health Assessment Tool 日本語版(OHAT-J)の口唇、舌、歯肉・粘膜、唾液、残存歯、義歯、口腔清掃、歯痛8 項目スコアと比較した。第1 に、GNRIスコアを目的変数、年齢、性別、OHAT-Jスコアおよび合計スコアを説明変数としてステップワイズ法(st-MRA)の重回帰分析を行った。第2 に、良好群のダミー変数を「0 」、不良群を「1 」の目的変数とし、st-MRAと同じ指標によりロジスティック回帰分析(st-LRA)を行った。【結果】義歯不良患者は21.9%であり、良好群3.0%と比較して有意に多かった(p=0.024)。義歯の不良(p=0.016)、唾液の湿潤不良(p=0.018)が低栄養を惹起する要因であった。【考察】OHAT-Jを用いた評価は、高齢患者の栄養管理状態を評価するのに役立つことが示唆された。【結論】義歯、唾液の2 指標を含む入棟時の口腔スクリーニングは、回復期リハ患者の栄養管理における重要な情報となる。\n\nObjective: This study aimed to identify the predictable indices of oral condition that can be used to evaluate the current nutritional status of patients in convalescent hospitals. Method: A total of 65 patients aged >65 years admitted in the convalescent rehabilitation ward of the General Rehabilitation Center Midori Hospital were included in the study. Nutritional status and oral status were assessed using the Nutritional Risk Index( GNRI) for older patients and the Japanese Oral Pain Assessment Tool( OHAT-J), respectively. The patients were divided into two groups, good nutrition group (GNRI score >92, 33 patients) and malnutrition group( GNRI score <92, 32 patients). Multiple regression analysis using the stepwise method( st-MRA) was performed with GNRI score as an objective variable and age, gender, OHAT-J score, and total score as explanatory variables. Binary logistic regression analysis( st-LRA) was performed with “0” as the dummy variable for the good nutrition group and “1” as the objective variable for the malnutrition group; the same indicators as those in st-MRA were used as explanatory variables. Results: The malnutrition group consisted of higher denture-deficient patients( 21.9%), while the good nutrition group consisted of significantly only a few patients (3.0% ; p=0.024). st-MRA showed that denture and saliva were responsible factors( p=0.016), while st-LRA showed that dry mouth due to saliva deficiency was a significant factor (p=0.018). Discussion: The results strongly suggest that the OHAT-J score is useful for evaluating the current status of nutritional management in older patients. Conclusion: Two items of the OHAT-J, denture defects and salivary deficiency, were useful for evaluating the current nutritional status of older patients in the convalescent stage.
