| 内容記述 | 本研究の目的はPost-NICU退院支援モデルの構築と適用課題の検討である。Post-NICU病床入室児家族で研究参加に同意が得られた6名に半構成面接を実施し、質的統合法（KJ法）を用いてPost-NICU病床入室児の在宅困難要因を解明した。次に在宅困難の構成要素と見取図の関係から、家族が欲している支援内容を抽出し、家族とともに看護師から提案する退院支援モデルを構築した。その結果、本モデルの支援項目は《生活しやすい住宅の確保》、《物品の準備》、《車両の改造》、《緊急時や医療機器取り扱いに係る知識 ・ 技術習得に向けた教育》、《経済的問題の解決》、《経済的支援のための情報提供の促進》、《家族 ・ 親族の協力》、《家族 ・ 親族以外のサポート体制》、《24時間利用可能な医療体制》、《訪問サービス》、《患児の成長発達に係る教育環境》、《患児の身体的 ・ 精神的体調コントロール》、《主介護者の身体的 ・ 精神的負担軽減》、《病院職員との関係調整》、《介護者支援ネットワークの活用》、《在宅療育への自信獲得》であった。本モデル適用の課題は、情報提供ができる看護師の育成と小児神経専門医とかかりつけ医の連携体制の構築、コーディネーターを担う在宅看護師の育成 ・ 配置であった。在宅介護への自信獲得には、母子愛着形成の促進と父親と児の関係性の確立、家族が揃って生活できる環境整備に向けた支援が必要といえた。 | |||||||||||||

| 内容記述 | The study objective was to construct a model for post-NICU discharge support, and consider application problems. We conducted semi-structured interviews with six families with infants hospitalized in post-NICU beds who consented to participate. Using a qualitative synthesis method（ K-J Method）, we elucidated factors related to the difficulties at home of infants hospitalized in post-NICU beds. Next, we extracted content on families’ desired support based on the relationships of these at-home difficulty components using an affinity diagram and constructed a discharge support model to direct and improve nurse-family interactions. The support items identified were as follows: livable housing, preparation of goods, modification of vehicle（s）, education for knowledge and technology acquisition related to emergencies and medical equipment handling, resolution of economic issues, promotion of information provision for economic assistance, cooperation from family and relatives, support system other than family and relatives, 24-hour medical care system, visitation services, educational environment in accordance with the children’s growth and development, control of the children’s physical and mental health, reduction in the physical and mental burden of primary caregivers, adjustment of relationships with hospital staff, caregiver support networks, and confidence acquisition in home care. This model can be applied to the training of nurses capable of providing information, construction of a cooperative framework for pediatric neurologists and primary care physicians, training and deployment of home nurses acting as coordinators, and the maximum use of existing resources. Actions deemed necessary to strengthen confidence in home care were promoting maternal attachment formation, strengthening the father-child relationship, and environment improvements allowing families to live together as a unit. |
