EPDSは産後うつ病のスクリーニング方法として活用されており、EPDSを用いた支援の研究も増加している。本研究の目的は、産後うつ病の評価指標として使用されているEPDSに焦点をあてて、産後うつ病への支援に関する研究の動向と支援内容、予防方法を分析し、看護の課題を明らかにすることである。医学中央雑誌web版を用い、「EPDS」AND「支援」で検索した、過去10年間の看護に関する原著論文34件を分析対象とした。EPDSを用いた支援に関する研究は、2009年以降増加し、2012年と2016年に7 件と最多であり、2010年以降からEPDSの継時的変化や継続的な支援に関する縦断研究が徐々に増加していた。スクリーニングの介入時期は妊娠初期から3 歳までであり、それぞれの時期に応じてEPDS以外の質問紙票を組み合わせて実施されていた。産後うつ病予防に関する支援内容は、【産後の支援の場と心理的支援】、【妊娠期からのスクリーニングと早期介入】が多く、母親の不安や悩みなど訴えの傾聴・支持や妊娠期における情報収集が重要であることが明らかとなった。身体的にも心理的にも不安定な産後は、病院助産師から地域母子保健の助産師・保健師による出産後のケア継続と、産婦健康診査事業の両輪で支援していくことが重要であると示唆された。 |

The Edinburgh Postnatal Depression Scale （EPDS） is commonly used as a screening tool for postnatal depression. Research on support for women during pregnancy or after childbirth using the EPDS is also increasing. The purpose of this study was to clarify nursing issues by analyzing research trends on support for postnatal depression, support content and prevention methods with a focus on the EPDS used as an assessment index for postnatal depression. Using the web version of the Igaku Chuo Zasshi, a search was conducted using “EPDS” AND “support” as keywords. A total of 34 original articles on nursing from the past 10 years were returned and these articles were analyzed. Studies on support using EPDS increased after 2009, with the largest number of 7 articles per year published in 2012 and 2016. Longitudinal studies on changes over time and continuous support in EPDS increased gradually from 2010. The screening intervention period was from early pregnancy until the child was 3 years old, and questionnaires other than the EPDS were used concurrently in accordance with each period. The most common support content related to postnatal depression prevention was “a place for postnatal support and psychological support” and “early intervention from pregnancy,” revealing that “listening closely to the anxieties and worries of mothers and giving support” and “information gathering during pregnancy” are important. After birth, when mothers are unsteady both physically and psychologically, support with both the continuation of postnatal care from hospital midwives to community maternal and child health midwives and public health nurses, and postpartum health checkup is suggested to be important. |
