本研究は、精神科認定看護師（Certified Expert Psychiatric Nurse：CEPN）の資格取得前の期待と取得後の評価を類型化し、専攻領域との関連を明らかにすることを目的とした。研究対象者は2014年のCEPN登録者559名とし、調査票の回収率は50.3％であった。28の質問項目の因子分析で得られた8 因子と総合評価指標等の総計12の評価指標を目的変数として段階式重回帰分析を行った。その結果、CEPN専攻領域の評価指標の強弱との関連は一様でなく、精神科看護経験年、学位（博士）、退院調整、精神科訪問看護、老年期精神障害看護が高度に有意であった。このうち老年期精神障害看護の領域は事後的評価が負であり、CEPNの寄与度が低く改善の余地が示唆される一方で退院調整、精神科訪問看護の両CEPNは資格が有効に機能し高く評価されていると判断された。いずれも、わが国において近未来の保健医療福祉の在り方の青写真である「地域包括ケアシステム」構想の影響によってこうしたCEPNの正負の動きが出ていると考察した。
The purpose of this study was to typify the pre-qualification expectation and post-qualification evaluation of certified psychiatric nurses （CEPN） and to clarify how these relate to the major field. For the current study, 559 CEPNs registered in 2014. Of these, the study questionnaire recovery rate was 50.3％ . The data were analyzed using stepwise multiple regression analysis. Eight factors were obtained by a factor analysis of 28 questions and a total of 12 evaluation indices as objective variables, such as comprehensive evaluation. Results indicated that the relationship between the strength of the evaluation index and the CEPNs major area was not uniform, but “Mental nursing experience year,” both “Master’s course graduates” and “Graduate doctoral course completion”, both “Discharge adjustment” and “Psychiatric nursing home visits,”, and “Geriatric Mental Health Nursing” were highly significant. Among these, the field of “Geriatric Mental Health Nursing” had a negative evaluation after the ex-post evaluation, and the contribution of CEPNs was low, suggesting room for improvement, while both “Discharge adjustment” and “Psychiatric nursing home visits,” had a positive evaluation after the ex-post evaluation, and the contribution of CEPNs was high. It is considered that all positive and negative movements of CEPNs appear under the influence of the “Community-based integrated care system” concept that shows the blueprint of the near future health care and medical welfare in Japan. |
